My very first clinical year (baby steps) :D

Hai blog. Saya dah masuk tahun ketiga dalam MBBS ni. Tak sangka rasanya masa cepat sangat berlalu. Rasa macam baru haritu je update entry membebel pasal zaman foundation hihi. Well life goes on and on and on and on (sambung sendiri). So, insya allah lagi 5 sem lagi sebelum graduation.

Nak cerita masa clinical year ni memang menarik. Dah start masuk hospital, jumpa patient, kena reject dengan patient, kena tengok drama houseman kena marah dengan medical officer, drama medical officer kena marah dengan specialist, kena jadi pendengar cerita hidup patient bila dia takde orang nak berborak, kena tolong suapkan makanan dekat patient sebab dia stroke and takde orang pon melawat dia dekat hospital, kena marah dengan dr bila kau tak clerk patient betul2, dapat jumpa case yang selama ni kau baca je dalam buku tapi ni kau nampak depan mata, kena hadap bau busuk masa tengok prosedur keluarkan nanah or bersihkan luka or anything and banyakkkkk lagi. (Well i'll be reading this entry in 3 years time as a houseman and laughing at myself).

What can i say is that i love the hospital surrounding so much. I am soooo envy when looking at how the doctors are struggling to figure out what is the diagnosis of the patient. I mean how come you can remember all the signs and symptoms of the diseases, interpret the investigation result and lastly come out with a diagnosis. Semua benda dah hujung jari, petik je, terus boleh diagnose penyakit patient and manage them. Aku yang student ni terkial kial lagi nak tanya apa dekat patient. Itu lupa, ini lupa. Nak buat physical examination pon tangan shaking gila lol. Jadi, saya berasa sangat jealous! So saya berazam dekat diri sendiri untuk belajar bersungguh sungguh nak jadi macam doktor dekat hospital yang sangat working hard tu insya allah aminnn.

So far dah masuk posting yang ketiga. First posting was OnG, second was Community Medicine (posting commed taknak cerita, kurang best) and currently in Internal Medicine posting. 2 more postings before the end of year 3 yeayyyyy. So far OnG is quite an easy posting for me compared to Internal Medicine posting because you'll be clerking using the same template and do the same physical examination. Unlike Internal Medicine, you need to know a lot of diseases since it involves all systems in our body. You need to know everything, like seriously everything.

For example, a patient come to you complaints of shortness of breath. So you need to think what diseases can cause it. What systems involve in it. You cant just assume someone with shortness of breath, he or she has problem with his or her respiratory system. It can involves cardiovascular system, GIT system, even people with diabetes also can have shortness of breath. Yeah so u can see u need to know everything and relate everything. A very strong pre-clinical knowledge is a must because it is a basic knowledge (which I forgot most of it).

Well there are most of the time when you feel so stupid and blur, you cant even understand the dr's handwriting on the patient's file, you have no idea what are the names of all the machines and wires and stuffs on the patient's hand, abdomen, chest, nose and mouth and what are their functions, you have no idea what are the doctors discussing about during the ward round, you are getting very confused about the disease of the patient, how come he can develop this, why is it like this, why is it not like this and many more. Trust me, it is a normal situation for you to have that stupid and blur feeling. I repeat, it is NORMAL. So, extra effort is needed. Tanya je nurses tu ni machine apa, untuk apa, kenapa patient ni kena pasang machine ni. Tanya houseman kenapa dia sakit ni, apa semua. They will never say no to you (cewah aku cakap macam aku rajin tanya je lol).

So, enjoy your clinical year :) Lain-lain hospital lain-lain cara. Paling relaks bila posting dekat Hospital Angkatan Tentera, Terendak. Patient tak ramai, dekat dengan pantai, memang berjimba je la cerita dia haha. Paling moden bila dekat Hosp Putrajaya. Paling best nak belajar and jumpa banyak case yang menarik and nak tengok how packed the ward is sampai extension bed dia penuh dekat tengah2 jalan dalam ward and patient sangat baik, dekat Hosp Kajang. Hosp Manjung pon lebih kurang mcm Kajang jugak tapi tak pack sangat, so tenang sikit la.

Anddd, the best thing bila dah borak2 dengan patient or family dia or dah habis clerk and buat PE dekat patient, diorang selaluuuu nasihat 'belajar rajin rajin', 'anak pakcik takde pon yang jadi doktor. Kamu belajar la rajin rajin ye' and selalu doakan 'semoga berjaya jadi dr nanti'. Insya allah, aminn untuk semua doa doa yang keluar dari mulut mereka. Semoga Allah sentiasa berikan kesihatan terbaik untuk mereka jugak :)

HIHIHI saja tulis panjang panjang sebab nanti lagi 3-4 tahun nak baca balik sambil senyum sorang sorang. Some people will feel very annoyed apesal la diorang ni gelabah sangat nak cerita semua benda, baru student, bukan dr pon. Well, to me, bukan nak show off sangat pon, masa student ni la kau excited experience benda baru and cerita so that kau tak lupa pengalaman kau. (yang kau taknak share cerita kau tu lantak kau lah, jeles letteww hihihi) Tapi agak agak la jugak kan takpayahla nak share semua benda. Ni baru cerita pasal belajar. Tak cerita lagi pasal gossip, pakwe makwe, kawen mawen (eh? haha). Nanti2 je lah

Nah sis belanja sikit gambar setakat 2 posting setengah ni hihi. Jangan cakap sis takpayuig pulak, Bukan selalu sis nak tunjuk gambar banyak banyak ni semua sekali tauuu

Selebet dont care

Sekian, dari nadzirah yang tetiba rajin menulis blog tetengah malam.
